 A critically reflective case study on independent prescribing for a patient with complex needs, which should be formulated around one of your prescribing logs demonstrating your decision making skills (3,500 words)

You should utilise the ten competency dimensions within the Royal Pharmaceutical Society, (2016) Competency Framework for All Prescribers. . 

 

Focus on a patient or client who you are working with and for who if you were able to prescribe, you would assess and make a prescribing decision.

Set the scene  Introduce the case study what are you going to be focusing on and why –i.e. telling us about the patient/client, the health condition(s)  and the patient’s care setting. 

Drugs which drugs are being taken including both any licensed and unlicensed medication and why they are being taken.  Remember this may include non-prescription drugs. What are you proposing to prescribe and why?

 

Analytical body of case study – analysing peer reviewed evidence – how did you assess / consult with the patient/ client, which clinical consultation models did you use and why, what is the evidence base?  Are there any other models or frameworks / tools you could have used and if so which ones?  What was the result of your assessment? Analyse the evidence base behind your prescribing decision.  Link to one of your prescribing logs.  You will need to critically analyse evidence demonstrating understanding of the competency domains within the Competency Framework for all Prescribers, (Royal Pharmaceutical Society, 2016) and their application to prescribing practice utilising a range of sources such as:

National and local guidelines

·        
Peer reviewed academic journals and text books taking into account the validity of the evidence

·        
Professional standards, competency frameworks and standards. 

 

Accountability & Responsibility – What as an IP (Independent Prescriber) what is your accountability and to who are you accountable.

When considering this, you will need to take demonstrate the following aspects:

·        
An understanding of the roles and responsibilities of an  Independent Prescriber 

·        
Legal and professional body accountability – to whom for what and what does the law say with regards to your future prescribing practice?

·        
Professional responsibilities as an IP – professional code and standards.

·        
Working as part of a multidisciplinary team.

·        
Communication with patients/service users carers and colleagues

·        
Public health considerations- safety, cost and effectiveness

·        
Other wider aspects that you might wish to specify

 

Demonstrate clear and detailed knowledge of the patient’s condition, the pharmacology of the drugs they are on. Any polypharmacy or polymorbidity issues need to be identified. Any allergies and sensitivities (has this affected medication choice, e.g. have you had to select second line treatment instead of first line and why is this so, i.e., is it because of allergies, or sensitivities? Address any ethical implications of your prescribing decision; ie driven by cost?

Demonstrate knowledge of social history and how this might affect concordance/adherence, evidence base around all of the above. Demonstrate how you came to a prescribing decision and how review/follow up the patient. This needs to include rationales for safety-netting and signposting and effective closure of the consultation. You might want to include strategies for patient partnership /decision-making / patient follow-up.

Conclude with evaluation of the outcome of your decision and any recommendations for further support for the patient; evaluate good practice and what could have been done differently.

Use evidence base throughout. Include robust analysis of evidence base: Mixture of local and national guidelines, peer reviewed texts and journal articles, legal and professional body standards.

Use the module learning outcomes as a guide to the required depth of your analysis.

You will need to demonstrate which learning outcomes you have achieved and these will need to be put at the bottom on your analytical account and in the mapping document.
