Video:

Consider what you learned from the video, along with other content that has been discussed in this course. For your initial post, choose ONE of the three scenarios presented below and answer the following questions. (Please include at least three scholarly sources within your initial post):

Questions:

– As an APRN, how could you positively affect the health of the woman in the scenario you chose?

– What would be the pros and cons of utilizing the technique of Motivational Interviewing with the woman in the scenario.

– Trial the techniques you have read about over the previous few weeks related to Motivational Interviewing with a coworker, family or friend. What was this experience like? Do you see benefits to becoming more comfortable with this technique? Why or why not? (Please, answer YES, and explain why?)

Note: Choose only ONE topic/scenario.

Scenarios:

1. A 34-year-old female who is 36 weeks pregnant – G2P1AB0 – presents to your office complaining of shortness of breath and ankle swelling. She states she has had “the flu” for several days and is insistent that this is why she can’t stop coughing. She is obviously short of breath when talking. In reviewing her chart, you can see she has a history of hypertension. She states she “”officially” stopped her blood pressure medications when she found out she was pregnant but admits that she rarely took her pills anyway, so “no big deal.”

2. Sumar has just found out she is pregnant and presents to the local free clinic. She is a 26-year-old G2P0AB1, and her first OB appointment is in two weeks. Sumar thinks she is about 12 to 14 weeks pregnant. She has come to the clinic complaining of indigestion. You notice the strong smell of cigarette smoke, and she admits she smokes one pack of cigarettes a day but states she is smoking less now since she started vaping.

3. You are seeing a 19-year-old female for STI screening. When reviewing her health and sexual history to determine her risk, she indicates she has had several new partners over the past six months and that she has not been using condoms but does have an IUD. She has started smoking marijuana and has dabbled with some other prescription medications over the previous two months. She is worried about having contracted an STI because she has noticed some “cauliflower” type bumps “down there” and would like to be tested.

This is a TPAL terminology for a better understanding of the above scenarios.

G = Gravida (number of pregnancies)

P = Para (Number of births after 20 weeks)

AB = Abortions

Introduction:
As an Advanced Practice Registered Nurse (APRN), it is crucial to understand how to positively affect the health of women in various scenarios. This includes utilizing techniques such as Motivational Interviewing to increase knowledge around the importance of preventive guidelines. In this post, we will focus on one scenario and discuss how an APRN can positively affect the health of the woman, the pros and cons of using Motivational Interviewing in this scenario, and the benefits of becoming comfortable with this technique.

Answer to Question 1:
In the chosen scenario of a 34-year-old female who is 36 weeks pregnant, an APRN can positively affect her health by implementing several strategies. Firstly, conducting a thorough assessment to identify any possible complications related to her symptoms of shortness of breath and ankle swelling is essential. This includes examining her cardiovascular and respiratory systems, as these symptoms could be indicative of preeclampsia or a pulmonary condition. Additionally, performing a comprehensive review of her medical history to identify potential risks associated with her hypertension and discontinuation of her blood pressure medications is crucial.

Furthermore, providing education on the importance of regular prenatal care and consistent medication adherence for chronic conditions, such as hypertension, is vital. The APRN can discuss the potential consequences of inadequate management of hypertension during pregnancy, including complications such as intrauterine growth restriction and preterm birth. Additionally, counseling the patient on the significance of lifestyle modifications, such as a balanced diet and regular exercise, to promote overall health and wellbeing during pregnancy is essential.

Answer to Question 2:
Using the technique of Motivational Interviewing with the woman in this scenario has both pros and cons. One advantage is that Motivational Interviewing can help the APRN establish a collaborative and non-judgmental relationship with the patient. This technique focuses on exploring the patient’s motivations, values, and goals rather than imposing the APRN’s agenda. It can empower the woman to express her concerns, desires, and barriers to adopting healthier behaviors, leading to increased motivation and self-efficacy.

However, there may be some challenges in utilizing Motivational Interviewing with this woman. One potential con is her lack of awareness or acknowledgment of the significance of her blood pressure medication adherence. The APRN may encounter resistance and ambivalence from the patient, as she may perceive her non-adherence as inconsequential due to her sporadic pill-taking history. Additionally, discussing the potential risks to her and her baby’s health without being judgmental or confrontational can be challenging, as the patient may interpret it as criticism.

Answer to Question 3:
Trialling techniques related to Motivational Interviewing with a coworker, family member, or friend can provide valuable insight into the benefits of becoming more comfortable with this approach. Practicing this technique fosters better communication skills, active listening, and empathy, which are essential aspects of patient-centered care.

The experience of using Motivational Interviewing techniques in personal relationships can be insightful. It allows the individual to understand the power of empathetic communication and the positive impact it can have on building trust, exploring motivations, and facilitating behavior change. By experiencing the benefits of Motivational Interviewing firsthand, the APRN can appreciate its potential application in a professional setting and recognize the value of supporting patients through collaborative discussion to elicit positive health outcomes.

Conclusion:
In conclusion, as an APRN, positively affecting the health of women in various scenarios requires comprehensive assessments, patient education, and the use of techniques such as Motivational Interviewing. By utilizing a patient-centered approach and considering individual motivations and barriers, the APRN can empower women to make informed decisions regarding their health and well-being. While there may be challenges and potential drawbacks of using Motivational Interviewing, the benefits of becoming more comfortable with this technique outweigh the potential limitations, enhancing patient-provider communication and facilitating behavior change.
