The civility index sheet will be attached later #1: Choose one activity from the civility index and list what you have chosen to improve upon. #2. List three measurable and actionable ways that you will improve this chosen item. #3. List the feedback from your partner in class in regards to how you can improve upon this goal. #4. List how you will measure this goal in 14 weeks and stay on track to continue to improve and become even better at this chosen goal. Include in each statement a reflection of this activity and how you felt doing self awareness, receiving and giving feedback.
